APPLICATION AND COLLEGE TRANSCRIPTS
Official transcripts from each educational institution you have attended since high school and the statement of participation are required for consideration for the EMBA program.

TRANSCRIPT REQUEST FORM
Complete a transcript request form and send it directly to each institution you have attended. Transcripts will then be forwarded to our office from those schools. You must submit an official transcript from each educational institution you have attended (or are attending) since high school. These transcripts are required regardless of your length of stay at the institution (including summer and graduate schools). Many schools charge a nominal fee for transcript requests. Please check with the institution regarding any fee you should include with each transcript request form you submit.

Number of transcripts to request
You must request two official copies of each transcript. If you have attended Georgia State, request the following:

• One copy of your Georgia State transcript (the EMBA office cannot request your Georgia State transcript on your behalf)
• One copy of your transcript from each institution attended before being admitted to Georgia State
• Two copies of transcripts from each institution you attended after attending Georgia State

Note: If you attended Georgia State, there is no fee to have your transcript sent to our program. However, you must request your Georgia State transcript from the Office of the Registrar. All transcripts submitted become the property of Georgia State University; they cannot be returned to you or transferred to another institution.

Note: If your status at Georgia State was transient or post-baccalaureate, submit two copies of transcripts from each institution.
