  • Your name as it should be displayed on your profile (e.g. Jane A. Smith)
  • Any additional position held, e.g. department chair
  • Email address to which a confirmation of this request and/or other correspondence regarding the request should be sent, e.g. a departmental staff member's email address, if filled out by another on behalf of a faculty member.
  • Faculty member's email address, if different
  • Physical location, e.g. office 1015, downtown campus
  • List each degree you want listed on your profile. Click the + at the end of each field to add an additional line.
