    Instructions:

  1. Double click on the form needed
  2. Select the "open" option
    (If gridlines are showing, go to "Tables", and select "Hide Gridlines")
  3. Type information directly into document
  4. Save document
  5. Email document as attachment to rcbhelp@gsu.edu or
  6. Print out a copy of saved document and drop off at
    RCB System Support office
